To apply for the Pharmacy Technician Certificate program, follow the steps below. If you have any questions, call us at (661) 654-2441 and we’ll be happy to assist you with your application.
Step 1 – Fill out application
The Pharmacy Technician Certificate program application is in PDF format. You can print it and fill it out by hand or fill in all the fields in Acrobat Reader, then print. Be sure to fill in all required fields, and be sure to sign and date at the bottom.
Step 2 – Application fee and submission
After you’ve completed your application, you’ll need to pay a $75 application fee (payable to CSUB by personal check, money order, or 3rd party authorization – no cash or credit cards accepted). Bring the application and fee to the Extended Education office, or mail it to us. Our address is:

Step 3 – Complete basic math test
After you’ve submitted your application and application fee, you’ll need to complete a basic math skills test in person at the Extended Education office. No appointment necessary – just stop by and check in at the front desk. The 24-question test should take you less than 30 minutes.
After you’ve completed the above steps, we’ll process your application and let you know a few weeks prior to the program start date about your admission status.
